These terms apply to purchases of the QTB Payer Fee Schedule & Medicare Benchmark Analyzer Professional Practice Edition from QTB Limited. By purchasing, downloading, accessing, or using the product, the purchaser agrees to these terms.
1. Digital Product and Delivery
This is a digital product. No physical item will be shipped.
The product package will be emailed to the address used during Stripe checkout, generally within one business day after successful payment. The purchaser is responsible for providing an accurate email address and checking spam or junk folders.

2. Professional Practice License
The purchase includes a limited, nonexclusive, nontransferable Professional Practice License for one purchasing organization.
The purchasing organization may:
• Use the workbook for its own internal administrative operations.
• Enter and analyze its own validated fee schedules, reimbursement data, and aggregate procedure-volume information.
• Share workbook results internally with its owners, employees, and authorized representatives.
• Create internal reports and summaries from its own analysis.
3. Prohibited Uses
The purchaser may not:
• Resell, redistribute, sublicense, copy, or publicly post the workbook or documentation.
• Share the files with unrelated organizations.
• Upload the files to a public or shared template library.
• Remove or alter QTB Limited copyright, license, or ownership notices.
• Claim the workbook, formulas, documentation, or design as its own product.
• Use the Professional Practice Edition across multiple unrelated consulting clients.
A separate consultant or multi-client license is required for use on behalf of multiple unrelated organizations when such a license becomes available.
4. Software and Source-Data Requirements
The product is designed for desktop Microsoft Excel. Compatibility with Google Sheets, Apple Numbers, mobile spreadsheet applications, or other software is not guaranteed.
No official Medicare or payer fee schedules are included. The purchaser is solely responsible for obtaining, entering, and validating all source data, including Medicare benchmarks, payer rates, contract terms, procedure volumes, and assumptions.
5. No Patient Information
The workbook is intended for aggregate CPT/HCPCS, reimbursement, and procedure-volume information only.
Do not enter protected health information, personally identifiable patient information, or direct patient identifiers. QTB Limited does not need or request patient information to provide product support.
6. Administrative Tool and Disclaimer
This product is an administrative analysis tool. It is not legal, coding, clinical, compliance, reimbursement, accounting, tax, contracting, or negotiation advice.
Workbook results depend on the accuracy, completeness, timeliness, and applicability of purchaser-entered information and assumptions. QTB Limited does not guarantee any particular reimbursement, contract increase, negotiation result, revenue improvement, or financial outcome.
Purchasers should validate important conclusions against current contracts, amendments, payer policies, official reimbursement sources, and qualified professional guidance before acting.
7. Support
Product support includes assistance with:
• Accessing the purchased files.
• Replacing a damaged or corrupted download.
• Reviewing a confirmed defect in the original product formulas or file structure.
Product support does not include:
• General Microsoft Excel training.
• Data entry or workbook completion.
• Medicare or payer-rate research.
• Validation of purchaser-entered information.
• Customized workbook development.
• Contract interpretation or negotiation.
• Reimbursement, coding, compliance, legal, tax, or consulting services.

8. Cancellations and Refunds
A purchaser may request cancellation before the digital product has been delivered.
Because access to a digital file cannot ordinarily be revoked after delivery, refunds are generally not offered after the product package has been emailed. QTB Limited will provide a replacement file or reasonable correction for a confirmed product defect and will review duplicate purchases or duplicate charges.
Nothing in this policy limits any non-waivable rights available under applicable law.
9. Intellectual Property
The workbook, formulas, documentation, graphics, product design, and related materials remain the intellectual property of QTB Limited. Purchase provides a license to use the product; it does not transfer ownership of the product or its intellectual property.
10. Limitation of Liability
To the maximum extent permitted by applicable law, the product is provided “as is” without guarantees of a specific business, financial, reimbursement, or contracting result.
QTB Limited is not responsible for losses resulting from inaccurate source data, incorrect assumptions, software incompatibility, unauthorized modification, failure to validate results, or decisions made using workbook outputs.
To the maximum extent permitted by applicable law, QTB Limited’s total liability related to the product will not exceed the amount paid for the product.
11. Updates
QTB Limited may update these terms or the product in the future. A purchase does not automatically include future editions, customized versions, consultant licenses, or newly developed products unless expressly stated.
